The Lead Nursing Operations Specialist (Director of Nursing) is responsible for assisting pharmacists in the performance moderately moderate complex technical and clerical duties involved in the daily operations of the Pharmacy, as allowed by law and/or regulation. This role will function as the Director of Nursing.
How you will make an impact:
Primary duties may include, but are not limited to:
Coordinates care with the interdisciplinary team, patient/family and referring agency.
Lead field staff to assure adherence to all policies and procedures and to federal, state and or/or local regulations.
Serves as liaison between clinical staff and operations staff; and liaison among the organization, hospitals, the medical community, and general community.
Coordinates and performs infusion nursing care for patients in the home setting; administers skilled nursing care; participates in teaching patients and caregivers on home medical management; provides leadership by working cooperatively to establish priorities and goals of patient care.
Provides infusion and other skilled nursing care to patients, including but not limited to: patient assessments, patient/caregiver teaching, development and revision of the plan of care, implementation of physician orders, monitoring patient response, responding to emergency patient needs, and psychosocial support.
Provides pre-discharge patient assessments in the hospital or clinic setting, when appropriate.
Supports the account executives and clinical liaisons by providing educational programs or visits to referrals sources, as requested.
Monitors QAPI program and assures appropriate corrective measures are performed; initiates joint projects and presentations to improve patient care and/or education of community members on issues pertinent to agency interests.
Participates in the activities of the organizations quality assurance program.
Serves as liaison between this agency, physicians, hospitals, and other agencies in the community regarding patient care and clinical matters.
Ensure proper collaboration between appropriate organization stakeholders and hospital case managers/discharge planners to assure the flow of patient care.
Serves as a lead and has oversight of complex clinical issues, may lead cross-functional teams, projects, initiatives, and processes.
May serve as departmental liaison to other areas.
Minimum Requirements:
Requires BS in nursing and minimum of 2 years of clinical specialty experience involving infusion therapy and patient assessment; or any combination of education and experience, which would provide an equivalent background.
Current unrestricted RN license in applicable state(s) required.
A minimum of one (1) year previous specialized home health experience preferred.
For Paragon business unit, satisfactory completion of a Tuberculosis test is a requirement for this position.

For candidates working in person or remotely in the below location(s), the salary* range for this specific position is $96,976 to $145,464 Annual
